Incident Report   - Violations of Code of Conduct - Athlete
(report can be downloaded at bottom of page)

The procedure for filing an Incident Report for Violations of Code of Conduct by an athlete is as follows:

Junior Athlete

Anytime a junior student represents a member school, he/she is under the jurisdiction of his/her local athletic association.  Incidents between competitors, between competitors and spectators,  between competitors and game officials, and between competitors and adults in positions of authority should be referred to the administration of the offending player(s) school.

Senior Athlete

Filing a complaint:

To be considered a valid complaint an Incident Report Form (Violation of Code of Conduct - Athlete) must be completed.  The Incident Report must cite the reason for the complaint and particulars of the alleged violation, including the naming of other parties involved (if applicable). The Incident Report Form, containing the complainant’s signature, must be faxed to the Fraser Valley Volleyball Commissioner  immediately.

Who can make a complaint?

An Incident Report can be filed by a Volleyball BC official  or  member  school coach who has information that leads him/her to believe a violation has occurred.

Notification

The Athletic Director from the school of the offending athlete will be notified immediately by the Fraser Valley Volleyball Commissioner.  Notification will be effected by a phone call followed by delivery of a fax or  email.   During the time that the complaint is being investigated the athlete will be suspended from competing.  (The Athletic Director or principal should notify the athlete of this.)  The Fraser Valley Volleyball Commissioner will collect written reports from the parties listed on the Incident Report Form and forward them to the school’s principal.

Resolution

The principal will investigate the complaint.  Once the principal contacts the Fraser Valley Volleyball Commissioner in writing (email or fax) stating that the matter has been dealt with to his/her satisfaction the athlete will be allowed to return to competition.

The Fraser Valley Volleyball Commission as a member of BC SCHOOL SPORTS recognizes that the administrative head of each school is held ultimately responsible in all matters in his/her school which concern interscholastic athletic contests.  The expectation of the Fraser Valley Volleyball Commission and BC SCHOOL SPORTS is that principals will ensure that Fraser Valley Volleyball Commission and BC SCHOOL SPORTS policies established for member school interscholastic athletics will be followed.
